The patient must meet the following criteria's:

• Diagnosis of a life limiting illness
• Prognosis of six months or less
• Patient agreement to non-aggressive treatment and seeks palliative care
• Residence in Broward, Dade or Monroe Counties
• Attending Physician licensed in the State of Florida
