Discharge Planning & Transfers, Patient and Family Education Patient and Family Education – Provided by all Hospice team members and is ongoing to help the patient and family feel comfortable with all aspects of hospice care. We have provided teaching tools for patients and families at the end of our book to help with your care and concerns. We know how important it is for our families to feel they are doing their best to care for those they love and these tools, we hope will help.
The Ethics Committee seeks to fulfill its commitment through case review, education and policy recommendations. It's fundamental principle is based on exploring the patient's optimal alternatives while preserving his or her right to make decisions. Additional, as an integral component of its philosophy, the committee supports the intellectual, spiritual and emotional integrity of health care professionals. Patients, Primary Caregivers, Patient/Family members, Professional Caregivers, Interdisciplinary Team members and Volunteers may consult with the Ethics Committee.
