Discharge Planning & Transfers, Patient and Family Education
Cultural Awareness, Ethics Committee

Discharge Planning & Transfers – Occasionally, patients transfer to a different facility or we discharge them from the hospice program if they are no longer appropriate for our services. Some reasons for discharge are the patient shows signs of improvement and has a greater life expectancy than six months or the patient requests aggressive treatment. In any event we will educate the patient and family on the reason for discharge or transfer and assist with alternatives and options.

Patient and Family Education – Provided by all Hospice team members and is ongoing to help the patient and family feel comfortable with all aspects of hospice care. We have provided teaching tools for patients and families at the end of our book to help with your care and concerns. We know how important it is for our families to feel they are doing their best to care for those they love and these tools, we hope will help.

Cultural Awareness – Recognized and respected by HospiceCare of Southeast Florida, Inc. We strive to care for our patients on an individual basis. We do not assume all persons will respond to a life-limiting illness in the same way. We ask that you help us in our awareness of your cultural needs and traditions so that we may serve you better.

Ethics Committee

HospiceCare of SE FL, Inc., Ethics Committee serves the patients, families, healthcare personnel and members of the community who may come across ethical dilemmas during their use of Hospice services. The Ethics Committee provides a forum for the case review in which different values, perceptions and information about treatment options can be discussed and assessed in an attempt to achieve a consensus that helps in the resolution of the ethical conflict.

The Ethics Committee seeks to fulfill its commitment through case review, education and policy recommendations. It's fundamental principle is based on exploring the patient's optimal alternatives while preserving his or her right to make decisions.

Additional, as an integral component of its philosophy, the committee supports the intellectual, spiritual and emotional integrity of health care professionals.

Patients, Primary Caregivers, Patient/Family members, Professional Caregivers, Interdisciplinary Team members and Volunteers may consult with the Ethics Committee. To request an Ethics Committee consultation call HospiceCare of SE FL, Inc. and ask to speak with the Director of Development or the Administrator (On-call).
