Patient, Family and Caregivers
As the unit of care for the hospice team, the patient, family and
caregivers provide vital information about their needs and the
methods acceptable to them in meeting these needs.
- Attending Physician – Patients designate an Attending Physician to manage their care.
- Hospice Medical Director – The Medical Director oversees the treatment by the hospice team, coordinates with the attending physician and is available for consultation.
- Hospice Nurse – Hospice Nurses coordinate the Individualized Plan of Care; provide specialized palliative care services and provide family teaching.
- Spiritual Counselors – The Hospice Spiritual Counselors assist in identifying the spiritual concerns of the patient and family the connection with a community of faith.
- Social Worker – Hospice Social Workers offer emotional support, counseling and community resource support services.
- Home Health Aide – Hospice Home Health Aides assist with personal care and light housekeeping services.
- Volunteer – Trained Volunteers provide a variety of services, including companionship for patients, family and caregivers.
- Pharmacist – Pharmacists work with the hospice team to ensure the appropriate monitoring, supervision and timeliness of the pharmaceutical services.
- Bereavement Counselor – Bereavement Counselors support he patient, family and caregivers throughout the dying process and offer follow-up grief education and support.
- Other Therapists and Counselors – Other therapies can be provided if the outcome will be beneficial for the patient and if approved by the physician.
